For Professionals
Derbyshire Dementia Information and Advice Service
Who are we?
Derbyshire Dementia Information and Advice Service (DDIAS) is commissioned by Derbyshire County Council and Derby and Derbyshire ICB to provide a service to people affected by dementia in Derbyshire. Run by Alzheimer’s Society, our team offer a service to people from the time that they start experiencing symptoms, and throughout their dementia journey. We can support the person with the symptoms, and/or their carer, including pre-diagnosis.
Colleagues in other services and provision across Derbyshire are welcome to refer (with their consent) people who are worried about their memories or have symptoms of dementia, and/or their carers, to our service.
If a referral is received before 4pm on a working day, we acknowledge receipt of the referral to the practitioner and make a courtesy call to the person referred that same day (or the next working day for referrals received later than this), with a fuller assessment and support starting within 10 working days.
You are welcome to re-refer people into our service at any time and as many times as required, as their situation and needs may change.
What do we offer to your patients/clients?
Our free service can provide information on dementia and advice on managing the condition, to help people with dementia plan for the future and stay independent for as long as possible, and to help support carers in their role. We also help people to navigate other available services to find the right support for them.
The benefit to NHS services/GP surgeries: our support may help reduce the non-medical demand on GPs and other medical staff.
If you would like more information about what we can offer your service, please contact our team by emailing derbyshire@alzheimers.org.uk or phoning 01332 208845 (Mon-Fri, 9am-5pm except bank holidays)
You’re welcome to download our service flyer to share with colleagues and clients within Derbyshire.
